Social Prescribing at Cherry Medical Practice

At Cherry Medical Practice, we are committed to supporting the holistic health and wellbeing of our patients. Alongside medical care, we work with the Health Improvement Team in Salford and the Wellbeing Matters programme to offer social prescribing services that address the non-medical factors that affect health.

What is Social Prescribing?

Social prescribing enables healthcare professionals to refer patients to a range of local, non-clinical services.

Recognising that health is shaped by social, economic, and environmental factors, social prescribing provides access to activities and support that help improve overall wellbeing. These may include:

  • Community and social groups
  • Arts, culture, and physical activities
  • Volunteering opportunities
  • Support services for lifestyle changes and independence

Health Improvement Team – Salford

The Health Improvement Team delivers services designed to promote healthier lifestyles and overall wellbeing. Their programmes focus on:

  • Physical activity
  • Healthy eating
  • Smoking cessation
  • Mental health and resilience

By working closely with individuals, the team helps set achievable health goals and provides ongoing support.

Wellbeing Matters Programme

The Wellbeing Matters programme, led by Salford CVS, connects people with community resources through a network of Community Connectors.

In Little Hulton, the Community Connectors are based on the precinct opposite the surgery. They provide personalised support and help patients access local activities and services that:

  • Reduce social isolation
  • Improve mental and physical wellbeing
  • Support independence
  • Assist in managing long-term conditions
